The LPN also assists with directing nursing care and supporting nursing personnel during the assigned shift.Responsibilities:Provide nursing care according to individualized care plans, physician orders, professional standards, and applicable regulationsAdminister medications and treatments safely and accuratelyComplete medication administration, treatment, and clinical documentation within required timeframesAssess and monitor residents within the LPN’s authorized scope of practiceIdentify and promptly report changes in resident condition to the appropriate supervisor, physician, and responsible partyAssist with resident admissions, transfers, and discharges, including required documentationParticipate in welcoming and orienting new residents and their families to the communityCoordinate care with nursing assistants, physicians, therapy professionals, pharmacy personnel, and other members of the interdisciplinary teamProvide direction, education, and support to nursing personnel during the assigned shiftServe as a clinical resource for nursing staff within the authorized scope of practiceAccurately identify and report skin changes and follow established skin-management protocolsObtain and report diagnostic test results in a timely mannerCount controlled medications at the beginning and end of each shift according to facility proceduresFollow pharmacy procedures for ordering, receiving, administering, storing, and documenting medicationsDocument incidents, accidents, adverse events, grievances, and concerns according to facility policyParticipate in facility surveys and inspections as assignedFollow infection prevention, safety, quality-of-care, and restraint-reduction proceduresProtect resident rights, privacy, dignity, and confidentialityRespond calmly and appropriately to changes in resident condition and other clinical situationsQualifications:Graduate of an approved practical nursing programCurrent, active Licensed Practical Nurse license in the State of MissouriCurrent BLS/CPR certificationAt least one year of skilled nursing or long-term care experience preferredStrong clinical judgment, communication, leadership, and documentation skillsDependable attendance and a strong work ethicAbility to adapt when resident needs or priorities changeAbility to work collaboratively with an interdisciplinary care teamProfessional and compassionate demeanorCommitment to treating every resident with compassion, dignity, and respectAbout Avalon View:Avalon View Health and Wellness is a skilled nursing, rehabilitation, long-term care, and memory care community located in Liberty, Missouri.
