Physical Therapist

Cooke School Transitions Program is seeking a part-time physical therapist to work one day per week. The hours are 8:30pm -3:45pm and it is a school calendar. Experience with developmental disabilities including Autism, Downs syndrome and learning disabilities is preferred.The physical therapist at Cooke Transitions conducts assessments of students ages 18-22 and establishes individual goals for improving mobility, balance, strength, posture and flexibility for a population with minimal to moderate disability. The goals of school-based physical therapy promotes safe and functional motor skills that can be generalized to school, home and community.Summary of Duties and ResponsibilitiesThe Physical Therapist, a Related Service Provider (RSP), is responsible for providing direct and indirect service to students and staff consistent with the integration and collaboration model of the Cooke School. In this capacity, it is expected that the Physical Therapist will establish collaborative relationships with parents, partner schools, and Cooke colleagues to ensure successful inclusion for Cooke students. The Physical Therapist’s responsibilities include:Assessment and Direct Service to StudentsInterpret assessments in order to determine level of functioning and needs of each childConduct ongoing informal assessments in the classroom and coordinate professional practice with pedagogic staffUtilize a variety of activities to meet students’ needs as determined by informal and formal assessmentUtilize knowledge of content area to effectively serve studentsMaintain ongoing communication with classroom teachers and parents about progress of the studentsAttend team meetings prepared with information about the students being discussedCollect and assess student homework for portfolio using portfolio guidelines and assessmentsMeet typical physical demands of therapeutic service provision as required for domainPlanning and Record KeepingDevelop Progress Reports on each student in conjunction with classroom teacher and other team membersCreate plans consistent with Progress Report goals for each childMaintain narrative logs for each childMaintain attendance records for each childComplete monthly documentation relevant to the number of sessions given to each childEnsure that make-up sessions are providedGeneral Professional ResponsibilitiesInitiate and foster good working relationships with teachers, paraprofessionals, administrators, and parentsAttend all planning meetings with the teachers and monthly meetings with each classroom teamParticipate in all Progress Report meetings convened at the school levelAttend and participate in all Cooke meetings and workshopsArrive at assigned site and meetings promptly and be prepared for daily assignmentPrepare documents, when necessary, for Committee on Special Education (CSE) meetings and, if applicable, impartial hearingsAdhere to carefully planned schedulesUtilize supervisor for guidance, discussion, collaboration, professional and self-growthDevelop professional growth goals with supervisor and create and implement a plan to reach the agreed-upon goalsStaff training is an integral part professional development at Cooke. Staff training is an integral part of the Cooke School and as such is a requirement of the job.
