A well-established community health organization in Elgin, Illinois is seeking a Board-Certified/Board-Eligible Internal Medicine Physician for a full-time, permanent opportunity. This role offers the chance to provide comprehensive primary care in a mission-driven, patient-centered setting.

• Full-Time | Permanent

• Salary Range: $219,300 – $280,000 annually (based on experience and qualifications)

As an Internal Medicine Physician, you will:

• Provide comprehensive outpatient primary care to adult patients

• Diagnose and manage acute and chronic medical conditions

• Perform preventive care services, screenings, and annual wellness exams

• Develop and implement individualized treatment plans

• Manage chronic conditions such as diabetes, hypertension, COPD, and heart disease

• Order and interpret diagnostic tests and labs

• Coordinate referrals to specialists when appropriate

• Collaborate with nurse practitioners, physician assistants, behavioral health, and care coordination teams

• Maintain accurate and timely documentation in the EMR

• Participate in quality improvement and value-based care initiatives

• MD or DO from an accredited medical school

• Completion of an accredited Internal Medicine residency

• Board Certified or Board Eligible in Internal Medicine

• Active Illinois medical license (or ability to obtain)

• Strong commitment to community-based and patient-centered care

• Competitive base salary: $219,300 – $280,000
