Social Worker (Broughton Hospital)

Job Classification Title Social Worker (NS) Grade NC11 Description of Work Broughton Hospital is seeking a Social Worker to support patients throughout hospitalization and discharge planning. The Social Worker assists the Clinical Social Worker and treatment team in determining the best course of treatment, develops discharge plans, coordinates community resources, and collaborates with patients, treatment teams, Management Care Organizations, and other agencies. The position will manage a caseload of psychiatric patients, some of whom may be dually diagnosed, and provide case management services and psychoeducation focused on skill building. Responsibilities Assist with completion of 24‑hour intake documents assigned to social work. Identify linkage to social services and resources based on patient needs while considering clinical assessments and supports. Identify appropriate private or residential placement resources. Complete residential placement referral applications, arrange pre‑discharge site visits, and coordinate service provider arrangements. Refer patients with housing needs to TCLI and coordinate with In‑Reach activities. Work within the multidisciplinary team and participate in associated meetings. Provide written updates to assigned unit social worker and supervisor regarding progress or barriers toward goals. Contact patients, advise them about available resources, and arrange appointments. Perform direct functions such as psychoeducation, maintain documentation in patient records, and arrange meetings and aftercare services. Collect information and prepare reports concerning patient progress. Update the treatment team regarding resource development and discharge efforts. Track and document placement inquiry outcomes in patient records to demonstrate consistent discharge planning efforts. Serve as liaison with social and health service providers, legal and/or housing resources, and others in the coordination and development of aftercare services. Qualifications Master’s degree in social work from an appropriately accredited institution; or Bachelor’s degree in social work from an appropriately accredited institution and one year of related experience; or equivalent combination of education and experience. Experience providing case management. Working knowledge of social work principles, theoretical base methods, and policy. Demonstrated ability to work effectively with interdisciplinary treatment teams and other departments. Preferred: Experience working with patients with mental illness, substance use, or intellectual or developmental disabilities; experience in an inpatient mental health facility; experience creating discharge plans. Conditional Requirements Selected applicants must pass a State Bureau of Investigation Criminal History Check and complete BLS/CPR and Crisis Prevention Intervention (CPI) training during orientation. Benefits – Sign‑On Bonus A sign‑on bonus of $4,000 may be offered to the successful applicant contingent on 2025‑2026 Budget Approval. To qualify for the sign‑on bonus, a newly hired permanent full‑time or full‑time part‑time employee must work 30‑40 hours per week; eligibility is prorated if hours are less than 40.
