Job Summary

We are seeking a dedicated and experienced Registered Nurse (RN) for a Home Health & Hospice Case Manager role in Rutland, Vermont. This position involves providing comprehensive skilled nursing care to patients in their homes, encompassing both home health and hospice services. The successful candidate will be responsible for managing a diverse caseload, ensuring high-quality, patient-centered care.

You will serve as the primary case manager, coordinating care with an interdisciplinary team, providing patient and caregiver education, and ensuring adherence to evidence-based practices and regulatory standards.

Key Responsibilities

• Utilize the nursing process to conduct comprehensive patient/caregiver assessments, develop individualized plans of care, implement nursing interventions, and evaluate outcomes in accordance with evidence-based practice and regulatory guidelines.
• Serve as Case Manager for an assigned patient caseload, actively managing schedules and productivity to meet agency standards.
• Complete accurate, timely, and compliant OASIS documentation, staying current with all State and Federal requirements and agency policies.
• Perform advanced nursing skills including IV use and maintenance, wound VAC management, and complex wound care.
• Ensure strict adherence to infection control standards throughout the plan of care.
• Collaborate effectively with Licensed Practical Nurses (LPNs), delegating tasks within their scope of practice and reviewing documentation to ensure continuity of care.
• Maintain up-to-date and accurate documentation of all nursing care provided, ensuring effective information integration for the healthcare team.
• Adhere to agency protocols for electronic health record (EHR) processes, including McKesson system for visit completion and follow-up notes.
• Provide comprehensive patient and caregiver education on medications, treatment options, discharge instructions, reportable conditions, and available services.
• Accurately identify and document inter-disciplinary patient needs (e.g., physical therapy, social work) and communicate this information to relevant disciplines in a timely manner.
• Complete a variety of Agency, State, and Federal forms (e.g., HHABN, Medicare Notice of Non-Coverage) accurately and promptly.
• Conduct visits in a time-effective manner and communicate scheduling needs to the Scheduler.
• Assess patient needs for Home Care Aide (HCA) services, develop and coordinate the plan of care with patient/caregiver, and provide ongoing supervision.
• Transmit patient incident reports (e.g., infections, injuries) to the Quality Improvement Specialist in a timely manner.
• Coordinate safe patient discharges and transfers, ensuring all necessary paperwork and patient information are processed according to policy and regulations.
• Collaborate with physicians, providers, team members, and vendors to support the recommended plan of care.
• Participate in monthly peer review audits, providing and accepting constructive feedback professionally.
• Proactively schedule, coordinate, and facilitate case conferences with the Manager and team members to ensure effective communication and documentation.
• Advocate for patient care issues to ensure the overall quality and type of care is sensitive to each patient/family's unique needs.
