Madonna Ptak Morton Plant Rehabilitation Center is a 120-bed, 5-star rated facility located on the campus of Morton Plant Hospital. We specialize in post-acute care and rehabilitation services, including physical, occupational, speech, and recreational therapy. Our mission is to provide superior, patient-centered care that enables individuals to regain independence and return home safely.

BayCare is seeking a compassionate and skilled Occupational Therapist with SNF experience to join our interdisciplinary team. The ideal candidate will provide high-quality, evidence-based occupational therapy services to patients recovering from neurological, orthopedic, cardiac, and complex medical conditions. You will play a key role in helping patients regain independence in activities of daily living (ADLs) and instrumental ADLs (IADLs).

Occupational Therapist Responsibilities:

• Conduct comprehensive evaluations and develop individualized treatment plans.

• Deliver occupation-based interventions focused on ADL retraining, transfer training, and functional mobility.

• Implement preparatory activities such as therapeutic exercise, balance retraining, and cognitive interventions.

• Collaborate with physical therapists, speech therapists, nurses, physicians, and case managers to ensure coordinated care.

• Educate patients and caregivers on adaptive equipment, home safety, and discharge planning.

• Document patient progress and outcomes in accordance with CMS and facility guidelines.

• Participate in interdisciplinary team meetings and contribute to care planning.

• Maintain productivity standards while ensuring ethical and patient-centered care.

Qualifications

• Current Florida license as an Occupational Therapist (OTR). Or eligible to obtain.

• BLS (Basic Life Support) required.

• Bachelor's degree Occupational Therapy required

• Minimum of 1 year experience in a Skilled Nursing Facility (SNF) or subacute rehab setting preferred.

• Knowledge of CMS guidelines and SNF documentation requirements.

• Strong clinical reasoning and communication skills.

• Ability to work effectively in a fast-paced, team-oriented environment.

• Experience with neurological, orthopedic, and medically complex patients is a plus.
