**Exciting Opportunity for Case Manager RN!** **Job Description:** - Join a dynamic team as a Case Manager RN in a Magnet Facility with Level II Trauma designation! - *Shift Details:* - Day Shift: 5x8-Hour (07:30 - 16:00) - Flexibility in shift times: 0700-1530, 0730-1600, 0800-1630, 0830-1700 - *Experience Required:* Minimum 2 years of acute care experience and Epic charting experience - *Key Responsibilities:* - Conduct comprehensive assessments and create individualized plans of care - Coordinate patient care and facilitate discharge planning - Perform continued stay reviews and determine medical necessity per evidence-based guidelines - Oversee prior authorizations and pre-certification reviews - *Patient Care:* - Work with diverse patient populations: neonates, adolescents, infants, adults, pediatrics, geriatrics - Manage patient ratios of 1:30 during days - *Collaborative Environment:* - Engage with interdisciplinary teams: IV Teams, Physical Therapy, Respiratory Services, Social Services, and more - *Required Certifications:* - AK RN License - Basic Life Support (BLS) - Preferred - ACM Certification - Preferred - *Additional Skills:* - Strong organizational skills and ability to multitask - Excellent communication skills, both verbal and written - *Availability:* - Weekend coverage required (every other weekend) - Flexible to float within case management services as needed - *Orientation:* - 24 hours of orientation provided - *Additional Requirements:* - Must have an active AK RN license for auto offer - Microsoft Office/Teams and Epic experience preferred **Why Catalytic Solutions (CatSol):** At CatSol, we connect dedicated therapists with rewarding assignments across the country.
