Field-Based RN Case Manager — Flexible Schedule & Impact
Adaptive Home Health
Adaptive Home Health is seeking a Registered Nurse (RN) Case Manager for field-based, full-time work in Texas. You will own patient assessments, document skilled visits (incl. OASIS), and deliver high-acuity home health care with a focus on wound care and infusion therapy. You will coordinate with physicians, families, and care teams to create and monitor the plan of care. You will advocate for patients, ensure continuity of care across disciplines, and drive timely follow-ups and barrier #J-18808-Ljbffr Adaptive Home Health
