If you meet the qualifications listed below and are interested, please apply now!Location: Georgetown, SC 29440Duration: 8 weeks with high possibility of extensionWork Schedule: DaysGuaranteed Hours: 40Responsibilities for this position include, but are not limited to:Evaluated residents to assess mobility, strength, balance, range of motion, and functional limitations.Developed and implemented individualized treatment plans to improve mobility, independence, and quality of life.Provided therapeutic exercises, gait training, balance training, transfer training, and functional mobility interventions.Treated residents recovering from orthopedic surgeries, strokes, neurological disorders, fractures, and other age-related conditions.Educated patients, families, and caregivers on home exercise programs, fall prevention, and safe mobility techniques.Recommended and trained patients in the use of assistive devices, including walkers, canes, wheelchairs, and prosthetics.Collaborated with physicians, nurses, occupational therapists, speech therapists, and interdisciplinary teams to coordinate patient care.Monitored patient progress, adjusted treatment plans based on functional outcomes, and documented therapy services accurately in the EMR.Performed discharge planning and provided recommendations for continued therapy and community resources.Ensured compliance with Medicare, Medicaid, state regulations, facility policies, and rehabilitation documentation standards.To be considered for this position, you should have:Active State Physical Therapist LicenseBLS/CPRTryfacta is an Equal Opportunity/Affirmative Action Employer.
