Assisted Living UAI/ISP Assessment Nurse

We are seeking an experienced and highly organized Assisted Living UAI/ISP Assessment Nurse to support the assessment and care-planning needs of our senior living community.

This full-time position reports directly to the General Manager and works collaboratively with the Executive Director, Sales Department, and Director of Nursing to prioritize workflow and help ensure timely, accurate assessments and successful resident transitions.

Key Responsibilities

• Complete pre-admission assessments for prospective residents before move-in, residents returning to the community following hospitalization, rehabilitation, and routinely scheduled and change-of-condition assessments.
• Develop, review, and update Individualized Service Plans (ISPs).
• Ensure UAIs, ISPs, and supporting documentation are accurate, complete, and compliant with Virginia assisted living regulations.
• Communicate assessment findings and service needs clearly and compassionately to residents, families, and community leadership.
• Collaborate with Sales and clinical leadership to support appropriate admissions and smooth move-in experiences.
• Identify changes in residents' conditions and communicate recommendations promptly.
• Maintain accurate documentation within PointClickCare.
• Manage multiple priorities and meet required assessment deadlines.

Qualifications

• Current, unrestricted LPN nursing license in the Commonwealth of Virginia.
• Current Virginia UAI and ISP certification.
• Previous assisted living experience required.
• Strong clinical assessment and critical-thinking skills.
• Experience using PointClickCare (PCC).
• Strong written and verbal communication skills.
• Ability to communicate professionally and compassionately with prospective residents, current residents, families, and interdisciplinary team members.
• Ability to work independently while remaining highly collaborative.
