Tryfacta is seeking an LPN /LTC for our client in Clifton Springs, NY 14432. This is a 13-week contract assignment. If you meet the qualifications listed below and are interested, please apply now!

Position Title: LPN /LTC

Location: Clifton Springs, NY 14432

Duration: 13 weeks

Work Schedule: Mid 5x8-Hour (15:00 - 23:00)

Pay rate: 40-43$/hr.

Responsibilities for this position include, but are not limited to:

• Provide direct nursing care to residents in a nursing facility/nursing home setting.
• Administer medications and treatments as prescribed by physicians and care plans.
• Monitor, assess, and document residents' health status using EPIC charting.
• Care for patients with degenerative disorders, including Alzheimer's, ALS, and Parkinson's disease.
• Perform clinical procedures, including obtaining and interpreting 12-lead EKG/ECGs.
• Implement behavior management techniques and respond appropriately to resident needs.
• Collaborate with healthcare teams, maintain patient safety, and float to other units within skill level when required.

To be considered for this position, you should have:

• Active New York State LPN/LVN License (required; pending licenses not accepted).
• Graduate of an accredited Practical Nursing (LPN) program.
• Current Basic Life Support (BLS) Certification.
• Experience working in a nursing facility or nursing home environment.
• Proficiency with EPIC electronic medical records (EMR) system.
• Strong knowledge of geriatric nursing and care of patients with chronic and degenerative conditions.
• Excellent communication, patient assessment, documentation, time management, and teamwork skills.
