A community-based healthcare organization in Eastern Oregon is seeking a full-time Social Worker to support patients, residents, and families across multiple healthcare settings. This position offers a varied scope involving case coordination, discharge planning, emotional support, community resources, care planning, and interdisciplinary collaboration. It is well suited for a Social Worker who enjoys working across different patient populations and care environments.

Responsibilities

• Assess patient and resident psychosocial needs
• Support discharge planning and appropriate placement when needed
• Provide therapeutic emotional support to patients and families
• Coordinate referrals to behavioral health, substance-use treatment, and other community resources
• Collaborate with case management, nursing, providers, and other members of the care team
• Participate in multidisciplinary care planning
• Assist patients and families with advance care planning
• Advocate for patient and resident needs and rights
• Maintain accurate and timely case-management documentation
• Develop and maintain relationships with community resources and partner organizations
• Participate in quality and performance-improvement initiatives
• Provide social work support across hospital, post-acute, community, and home-based care settings as needed

Minimum Qualifications

• Bachelor's degree in Social Work
• Valid driver's license or permit
• BLS certification, or ability to obtain within 60 days of hire
• Strong communication, organization, and interpersonal skills
• Ability to collaborate effectively with patients, families, healthcare professionals, and community resources

Preferred Qualifications

• Master's degree in Social Work
• Licensed Clinical Social Worker (LCSW)
• Previous experience in hospital, post-acute, long-term care, hospice, or similar healthcare settings
