We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our home care team, providing quality and personalized health care services to our patients in Manhattan. In this role, you will be responsible for conducting patient visits in their homes, ensuring they receive the highest level of medical care and support. **Key Responsibilities:*

- Conduct comprehensive assessments of patients’ health status, and update care plans accordingly.

- Conduct supervisory visits to update the care plans and ensure they are followed.

- Educate patients and their families on health maintenance and disease prevention.

- Collaborate with interdisciplinary team members such as  social workers and other case managers to deliver coordinated care.

- Document all patient interactions, treatments, and symptoms in an accurate and timely manner.

- Respond to patient inquiries and concerns with a professional and caring demeanor.

- Maintain confidentiality of patient health information in compliance with HIPAA regulations.

**Qualifications:**

- Current and valid LPN license in the state of New York.

- Experience in home care or community health settings preferred.

- Excellent interpersonal and communication skills.

- Ability to work independently and manage time effectively.

- Strong critical thinking and problem-solving abilities.

- Good knowledge of Manhattan subway and bus systems
