We're seeking a Clinical Assessor to support the Kansas Home and Community-Based Services (HCBS) Program in the Colby, Kansas area.

Through comprehensive assessments, our team helps determine eligibility and connects individuals to programs and resources designed to support their unique needs.
As a Clinical Assessor, you’ll play a crucial role in
- Conducting comprehensive in-person assessments to evaluate participant needs, functional abilities, and eligibility for services.
- Gathering and documenting information from participants, caregivers, and providers to support accurate and timely determinations.
- Educating individuals on available programs, resources, and support options to help them make informed decisions.
