Description Concern Housing is seeking a Home Health Aide to provide hands-on, person-centered support to aging residents living in congregate care mental health settings. The HHA plays a critical role in supporting residents' health, dignity, independence, and quality of life by assisting with activities of daily living, personal and environmental hygiene, and routine household tasks in accordance with individual care plans. Working closely with clinical and residential staff, the HHA helps promote safe, person-centered care and supports individuals with complex medical and mental health needs. Position will be based out of East Patchogue but will also service clients located in Sayville. Working closely with clinical and residential staff, the HHA helps promote safe, person-centered care and supports individuals with complex medical and mental health needs. Position will be based out of East Patchogue but will also service clients located in Sayville. Days/Hours Monday - Friday or Tuesday - Saturday or Sunday - Thursday; 9am - 5pm Duties And Responsibilities Provide direct assistance with activities of daily living (ADLs), including personal hygiene, grooming, showering, and dressing. Support residents with environmental hygiene tasks such as laundry, light housekeeping, and maintaining clean, safe and organized living spaces. Assist with meal preparation, grocery shopping and basic nutrition support. Observe and report changes in residents’ physical condition, mental health status, or functional abilities to supervisory or residential staff in a timely manner. Communicate respectfully and effectively with residents and interdisciplinary team members. Encourage residents’ independence, autonomy and engagement while providing appropriate assistance and support. Follow all agency policies, procedures and regulations related to confidentiality and safety. Maintain accurate and timely documentation in residents’ electronic health records, if assigned. Attend weekly supervision. Remain current with all agency required compliance documents and trainings. Qualifications High School Diploma or Equivalent Home Health Aide Certification from a NYS Department of Health approved training program Two years of experience working with adults with serious mental illness, chronic medical conditions, aging populations, or a combination of these Strong skills in collaboration and communication. Willingness and ability to travel between East Patchogue and West Sayville.
