Job Title: Hospice Nursing Case Manager Profession: Registered Nurse Specialty: Hospice Duration: 13 weeks Shift: Day Hours per Shift: 8 hours Experience: Minimum years of hospice case management experience required. License: Active RN license required. Certifications: BLS certification required. Must-Have: Experience in home hospice case management is required. Description: We are seeking a compassionate and dedicated Registered Nurse to serve as a Hospice Nursing Case Manager. The position involves managing a caseload of patients in home hospice settings. The average daily census for this office is approximately 35-45 patients. The RN will perform visits in both private homes and facilities. The role includes conducting initial evaluations, follow-ups, and supervisory visits. The RN is required to have experience with the EPIC charting system. Candidates must be able to work collaboratively as part of an interdisciplinary team. The RN will be responsible for ensuring the comfort and dignity of patients at end-of-life stages. Your expertise will play a critical role in delivering quality care and support to both patients and their families. Experience with patient assessments and care planning is essential. Candidates should be prepared to handle the emotional challenges that come with hospice care. This position may require weekend coverage; details will be discussed during the interview.
