Job Summary:

William Penn Nursing and Rehab, a premier skilled nursing facility located in Lewistown, Pennsylvania, is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our interdisciplinary team. As an LPN at William Penn, you will have the opportunity to work with a diverse patient population, providing high-quality care and support in a warm and welcoming environment.

Responsibilities:

• Provide direct patient care, including wound care, medication administration, and vital sign monitoring, under the supervision of registered nurses and physicians

• Collaborate with interdisciplinary teams to develop and implement individualized care plans that meet the unique needs of each patient

• Perform routine nursing tasks, such as bathing, dressing, and grooming patients, as needed

• Document patient progress, medication administration, and treatment plans accurately and thoroughly in medical records

• Participate in quality improvement initiatives to ensure the highest level of patient care and satisfaction

• Promote a positive and respectful work environment, fostering open communication and teamwork among colleagues

• Additional responsibilities may include escorting patients to treatments, performing vital signs, and assisting with mealtime and daily living activities
