Care Coordinator Pregnancy & Family Support (I / II / III)

Department: ELEO Family Center

Reports To: Program Manager, ELEO

MACC exists to glorify God and share the good news of Jesus Christ through support, cooperation, and charitable works among associated members and participants. All social-service programs are led by faith-based principles, offering neighbors tangible help alongside spiritual hopeespecially in life-shaping seasons such as pregnancy and early parenting.

Position Summary

The Care Coordinator (I/II/III) walks alongside pregnant and parenting neighborshelping them love their neighbor as themselves by integrating health, social-service, and spiritual supports. Core duties include assessment, care-plan development, resource navigation, pregnancy & postpartum education, spiritual encouragement, and data tracking. Higher tiers carry progressively larger caseloads, leadership of junior staff, program improvement, and community partnership building.

Essential Duties & Responsibilities

Holistic Intake & Assessment

  • Conduct biopsychosocial + spiritual intake (pregnancy status, faith preferences, social factors).
  • Determine tier-appropriate caseload (Tier I 2734, Tier II 3547, Tier III 4650 neighbors).

Care-Plan Development

  • Co-create individualized plans covering prenatal/medical care, nutrition, transportation, housing, mental health, and spiritual care.
  • Document in EMR / case-management system within 24 hrs.

Pregnancy & Postpartum Support

  • Provide evidence-based education (trimester milestones, labor prep, breastfeeding, newborn care).
  • Coordinate doula services, childbirth classes, and referrals to perinatal specialists.

Spiritual & Faith Integration

  • Offer prayer, Scripture encouragement, or pastoral referrals consistent with MACCs Statement of Common Understanding.
  • Link families to local churches as trusted sources of community and support.

Resource Navigation & Advocacy

  • Secure Medi-Cal benefits, WIC, food bank deliveries, diapers, transportation vouchers, and safe-housing options.
  • Advocate with clinics, insurers, and social-service agencies to remove barriers.

Community & Stakeholder Engagement (Tier II/III)

  • Build partnerships with OB practices, pregnancy-resource centers, and churches.
  • Lead outreach events promoting holistic prenatal care.

Mentoring & Leadership (Tier II/III)

  • Orient new Care Coordinators; review charts for accuracy.
  • Serve as county lead, coach Tier I staff, identify workflow improvements, and lead case-round reviews.

Required Qualifications

All candidates must demonstrate a commitment to MACCs faith-informed mission and the ability to support pregnant and parenting neighbors with compassion and integrity.

  • Tier I:
  • - High school diploma or GED; and
  • - At least one (1) year of direct client service, case management, outreach, or ministry-related support experience.
  • - Entry-level care coordination. Familiarity with pregnancy-related social services preferred.
  • Tier II:
  • - Associates degree in health sciences, human services, ministry, or related field; OR
  • - High school diploma/GED and two (2) additional years of relevant experience.
  • - Able to mentor Tier I staff. May lead spiritual support groups, prenatal education, or volunteer teams.
  • Tier III:
  • - Bachelors degree in social work, public health, theology, or related field; OR
  • - Associates degree and two (2) additional years of senior-level care coordination or program oversight experience.
  • - County lead and subject matter expert. Participates in program evaluation and design. Strong experience in pregnancy ministries or prenatal resource programs preferred.

Note: Experience may be substituted for education on a year-for-year basis, subject to MACCs discretion.

Competencies & Skills

  • Strong knowledge of pregnancy/parenting resources and CalAIM Community Supports
  • Trauma-informed, culturally responsive, bilingual Spanish/English preferred
  • Proficient in Microsoft 365, SharePoint, EMR/case-management platforms
  • Able to travel across Colusa, Glenn, Tehama, Butte, Shasta, Yuba, Sutter & Yolo counties (vehicle provided as needed)

  • Affirms MACCs Statement of Common Understanding and integrates Christian principles in daily work.
  • Respects the dignity of every neighbor, regardless of faith stance, choosing compassionate service over coercive proselytization.

Working Conditions

Hybrid office/field role; evening or weekend classes/events as scheduled. Extensive county travel; ability to lift 25 lbs of program materials. Reasonable accommodations provided as required.
