Join us on this meaningful journey where your skills, compassion, and dedication will make a remarkable difference in the lives of those we serve.How you'll contributeA Clinical Liaison who excels in this role:Builds and maintains referral relationships to drive appropriate admissions to the hospitalAssists and coordinates in-take and pre-admission screening processPerforms on-site (typically in referring facility) clinical judgment to determine appropriateness of patient for admissionCommunicates to patients and family's rehabilitation and facility options. Communicates to patients determined appropriate for admission the patient's expectations and requirements for participationSecures information relating to patients' resources and benefitsFunctions as initial contact for external case management and discharge planner personnel with payors and hospital facilitiesWhy join usWe believe that investing in our employees is the first step to providing excellent patient care.
