- Conduct thorough assessments of patients in both inpatient and outpatient settings and craft personalized rehabilitation plans
- Deliver a wide range of physical therapy interventions focused on mobility, endurance, strength, and overall function
- Work in tandem with an integrated team of nurses, physicians, case managers, and rehabilitation professionals to ensure optimal patient outcomes
- Guide patients and their families in understanding treatment objectives, discharge processes, and recommended home exercises
- Record evaluations, treatment procedures, and patient progress adhering to facility and relevant regulations
- Engage in interdisciplinary team meetings and care rounds to enhance coordinated patient care
- A Doctorate in Physical Therapy (DPT) or a comparable degree recognized by an accredited institution
- Valid state license as a Physical Therapist
- CPR certification is advisable
- Specialized certifications (such as neurology or geriatrics) are advantageous, though not mandatory
- Previous relevant experience is strongly preferred
- Capability to assist patients with intricate medical histories and mobility challenges

- Proficiency with electronic charting and clinical documentation tools
