Adult Therapist

Job description:

Vital Health provides integrated primary care, behavioral health, and substance use treatment for adults and children in Northwest Ohio. Our mission is to improve the lives of our patients, families, and community through individualized and holistic care.

Position: Full-time (Mon-Fri),

Requirements:

LSW/LISW/LPC/LPCC (required or eligible)
Bachelor's degree required; Master's preferred
Experience in substance use and mental health treatment
Knowledge of community resources and referral systems
Understanding of confidentiality, patient rights, and professional ethics
Ability to work as part of a multidisciplinary team
Responsibilities:

Complete diagnostic assessments for new admissions
Diagnose SUD and co-occurring disorders within scope of practice
Develop treatment plans and reviews
Provide individual and group counseling (PHP/IOP/Day Treatment)
Collaborate with case managers to connect patients to resources
Conduct home/community visits when needed
De-escalate patient concerns and maintain professional boundaries
Document services in the EHR
Ensure compliance with confidentiality and privacy regulations
