Key Responsibilities

• Evaluate and treat patients across multiple care settings as part of a split assignment
• Develop and implement individualized physical therapy treatment plans
• Monitor and document patient progress toward established therapy goals
• Collaborate with interdisciplinary care teams to ensure continuity of patient care
• Maintain accurate and timely clinical documentation in accordance with facility standards

Required Qualifications

• Active Physical Therapist (PT) license in the state of Montana or ability to obtain prior to start
• Graduate of an accredited Physical Therapy program
• Ability to work a Monday–Friday daytime schedule
• Willingness to work across multiple care locations as part of a split assignment

Preferred Experience / Skills

• Prior experience in a skilled nursing or rehabilitation setting
• Experience managing a caseload across multiple sites or units
• Strong organizational and time management skills
