• Monday-Friday
• 6-8 Patient-Facing Hours Per Day
• Approximately 2 Additional Hours of Administrative / In-Basket Work Per Day
• 30-Minute Unpaid Lunch
• No Weekend Coverage
• No Call
• Provider May Work Across Primary Care Offices in Farmington

• Primarily Adult Patients
• At Least 16 Patient Visits Per Day

• Provide Ambulatory Primary Care
• Evaluate and Treat Acute and Chronic Medical Conditions
• Perform Annual Wellness Visits
• Provide Post-Hospitalization Follow-Up
• Manage Routine Primary Care Needs
• Manage Prescription Refill Requests
• Respond to Patient Advice Requests
• Review and Follow Up on Diagnostic Testing and Results
• Manage Referrals
• Complete In-Basket and Other Administrative Responsibilities
• No Specific Procedures Required

• Outpatient Primary Care Practice
• Approximately 12 Physicians and 15 APPs Across the Practices
• Strong Physician and Clinical Team Support
• No Residents or Fellows
• Epic EMR

• Board Certified - Required
• Family Medicine Training - Required
• At Least 2 Years of Clinical Experience in Primary Care - Required
• Active Maine Medical License - Required
• Epic Experience - Required
• COVID-19 Vaccination - Required
• Flu Vaccination - Required
• 2 Supervisor References and 1 Peer Reference From the Previous 12 Months - Required
• Provider Must Be Internally Credentialed With Presenting Agency

• Full Presentation Required at Time of Name Clearance
• Current Rates Must Be Included With Presentation
• Epic Experience Must Be Confirmed
• COVID-19 and Flu Vaccination Status Must Be Confirmed
• Years of Locum Experience Must Be Provided
• Overall Years of Clinical Experience Must Be Provided
• 2 Supervisor References and 1 Peer Reference From the Previous 12 Months Required
• Professional Licensure, Employment, Medical Staff Privilege, and Professional Conduct History Must Be Disclosed as Required
