Clinical Pharmacist

St. Vincent de Paul Cincinnati (SVDP) has served residents of Greater Cincinnati for over 150 years with basic needs such as food, medicine, and homelessness prevention through its outreach center in the West End and 53 parish-based volunteer Conferences. SVDP provides holistic direct assistance and systemic change services while treating neighbors in need with dignity and compassion.

Scope of Position: The clinical pharmacist provides direct patient through Comprehensive Medication Management and Disease State Management services at multiple sites of the Charitable Pharmacy. In addition, this position serves as a patient advocate, ensuring each patient's eligibility for services and connecting patients to resources that affect their social determinants of health. This position also serves as a preceptor for pharmacy students and residents collaborating to provide direct patient care.

Job Responsibilities:

• Provide clinical pharmacy services, including comprehensive medication management, diabetes self-management education, tobacco cessation, and other disease state management services
• Manage outcomes data related to program goals and objectives
• Assist Director of Pharmacy in pharmacy training programs, including Introductory and Advanced Pharmacy Practice Experiential students and post-graduate year 1 residents.
• Certify patients for Charitable Pharmacy services, obtain complete information for audit readiness, connect patients to necessary resources and advocate to fulfill patient-specific needs, and perform data entry of all patient information in various databases.

Qualifications:

Registered pharmacist in Ohio, doctor of pharmacy degree from accredited college of pharmacy. Strong organizational, attention to detail, interpersonal, written and oral communication. Experience in healthcare databases and data management. Ability to work with people of diverse backgrounds in a faith-based setting.

Work Environment: Staff member will primarily work in an office environment, adequately lighted, heated and ventilated at multiple locations within Greater Cincinnati. Physical Requirements: Walking, standing, bending, carrying items up to 35 lbs, driving an automobile
