**Please note: In fall 2026 the Pennsylvania Psychiatric Institute is relocating inpatient services in Harrisburg Pennsylvania to Holy Spirit Medical Center in Camp Hill. Our team is working closely with the Holy Spirit Medical Center Behavioral Health team and the Penn State Health Department of Psychiatry and Behavioral Health to ensure a smooth and coordinated transition .

SUMMARY OF POSITION:

Responsible for coordinating the individualized care continuum for patients and for achieving quality cost-effective and patient-focused outcomes by serving as an advocate educator consultant leader and community ambassador for a specific patient population in accordance with standards and guidelines. Provides consistent care coordination throughout the continuum of care and is able to assess the physical psychological and social needs of the patient. Work is strongly focused on guiding the patient/family through the complex healthcare system working to eliminate barriers that might otherwise adversely impact patient care/outcomes and helping to coordinate the efforts of the medical team to facilitate continuity of care and enhance patient satisfaction.

MINIMUM QUALIFICATION(S):

• Bachelors degree in a human services field
• Minimum two (2) years related experience
• Non-Violent Crisis Prevention training required prior to end of orientation
• Basic Life Support (BLS) required prior to completion of orientation
• PA Act 31 Child Abuse Training Certificate.
