Geriatrician / Internal Medicine Physician Center for Healthy Aging

CHRISTUS St. Vincent | Santa Fe, New Mexico

CHRISTUS St. Vincents Center for Healthy Aging is seeking a compassionate and dedicated Geriatrician or Internal Medicine Physician to join our growing outpatient practice. Opportunity Highlights

• 100% outpatient practice
\n
• Flexible schedule: \n
  \n
  • MondayFriday, 8-hour days, or Four 10-hour workdays per week
  \n
\n
• Manageable patient volume of approximately 16+ patients per day
\n
• Limited call responsibilities: Primary Care call rotation, telephone call only with nurse triage support
\n
• Focus on providing comprehensive care for older adults in a supportive, team-based setting
\n
• Epic health record
\n
• Robust Nurse Navigation support
\n

Position Responsibilities

The physician will provide comprehensive outpatient medical care for adult and geriatric patients, including:

\n
• Preventive health screenings and wellness evaluations
\n
• Diagnosis and management of acute and chronic medical conditions
\n
• Treatment of common geriatric conditions such as: \n
  \n
  • Dementia and cognitive disorders
  \n
  • Osteoarthritis and musculoskeletal conditions
  \n
  • Urinary incontinence
  \n
  • Multiple chronic disease management
  \n
\n
• Development and oversight of individualized treatment plans
\n
• Coordination of care with specialists, caregivers, and interdisciplinary team members
\n
• Ordering and interpreting diagnostic tests and procedures
\n
• Supporting patients in achieving their health goals while preserving function and quality of life
\n

Qualifications

\n
• MD or DO
\n
• Board Certified or Board Eligible in: \n
  \n
  • Geriatric Medicine, or
  \n
  • Internal Medicine
  \n
\n
• Eligible for or currently holding a New Mexico medical license
\n
