If you're looking for a flexible schedule, meaningful patient relationships, and the opportunity to make a lasting impact every day, we'd love to meet you.Your Key ResponsibilitiesConduct comprehensive psychosocial assessments to identify patient and family needs, develop individualized care plans, provide interventions, and monitor progress toward patient goals.Deliver skilled medical social work services in accordance with physician orders and the CMS Conditions of Participation (CoPs).Maintain timely, accurate documentation while ensuring compliance with CMS, payer, and agency standards.Collaborate with patients, caregivers, physicians, and the interdisciplinary team to promote safe, effective care and successful discharge planning.Provide counseling, emotional support, caregiver education, crisis intervention, and referrals to community resources, financial assistance, transportation, Durable Medical Equipment, long-term care, and other support services.Advocate for patients, address psychosocial barriers to care, and communicate changes in patient status to the care team.
