Social Services Director (Skilled Nursing / Long-Term Care)

Make a Meaningful Impact in Long-Term Care Brownsburg Health Care Center is seeking an experienced Social Services Director to lead psychosocial services in our Skilled Nursing Facility (SNF) . This role is ideal for a licensed social worker passionate about resident advocacy, care planning, discharge planning, and interdisciplinary collaboration in a long-term care setting. Key Responsibilities Complete comprehensive psychosocial assessments for residents Develop and implement individualized care plans Provide emotional, social, and mental health support to residents and families Lead discharge planning, transitions of care, and community resource referrals Advocate for resident rights, dignity, and quality of life Collaborate with nursing, therapy, and interdisciplinary care teams Maintain accurate documentation in compliance with state and federal regulations Participate in state surveys, audits, and compliance reviews Supervise or support social services staff, as applicable Required Qualifications Bachelor’s degree in Social Work or related Human Services field (Psychology, Sociology, Human Services, Rehabilitation Counseling) Licensed, certified, or registered to practice in Indiana Minimum 1 year of supervised social work experience in a healthcare setting Knowledge of CMS regulations, resident rights, and discharge planning processes Preferred Qualifications 2+ years of experience in a Skilled Nursing Facility (SNF) or Long-Term Care (LTC) environment Previous Social Services Director or leadership experience Benefits & Perks Competitive pay Medical, Dental, and Vision Insurance Paid Time Off (PTO) 401(k) retirement plan Life insurance options Supportive management and stable long-term care environment Why Choose Brownsburg Health Care Center?
