Care Coordinator (Remote)

:

Position: Care Coordinator ( Remote) Division: Mental Health Division Schedule: Monday - Friday 9am - 5pm Travel: 40% (Travel Reimbursed) Hourly Rate: $22

Location: Bronx NY, - Must do client visits 1-2 days within a week, You arrange the visits.

Requirements :

• Bachelor's Degree in Human Services or related degree
• Minimum (2) years of experience providing direct services to clients suffering from mental health, substance abuse, or homelessness
• Proof of Full Covid-19 Vaccination
• Bilingual Spanish a plus

Responsibilities :

• Responsible for individual caseload of 25-30 adult clients, each of which is to be visited face-to-face once a month.
• Assure that clients attend medical appointments at outpatient clinics as well as comply with medication regimens.
• Helps participants obtain benefits including food stamps, ongoing financial and housing needs, health care
• Initiate appropriate referrals to additional services as needed.
• Facilitate and participate in client Case Conferences
• Immediately report serious incidents, allegations, and complete incident reports
• Write clear Progress notes of services and visits in the patient's electronic medical record, within 48 hours.

Job Type: Full-time

Pay: $22.00 per hour
