LPN or RN MDS Coordinator

Job Summary:

Blue Palms Health and Rehabilitation Center at Fletcher is seeking a dedicated and detail-oriented LPN or RN MDS Coordinator to join our interdisciplinary team in Tampa, FL. As a seasoned professional in the field of long-term care, you will play a pivotal role in ensuring our residents receive the highest level of care and services. Responsibilities:

• Coordinate and conduct thorough assessments of residents' medical, social, and psychological needs to develop and implement comprehensive care plans in collaboration with the healthcare team

• Review and update Minimum Data Set (MDS) assessments, care plans, and resident records to ensure accuracy and compliance with regulatory requirements

• Collaborate with caregivers, physicians, and other healthcare professionals to provide high-quality care and services that meet the unique needs of each resident

• Identify and report any changes in residents' conditions, concerns, or complaints to the healthcare team and family members

• Develop and maintain effective communication systems to ensure seamless coordination of care and services

Requirements:

• Current LPN or RN licensure in the State of Florida

• Minimum of 1 year of experience in long-term care, gerontology, or related field

• Strong clinical skills and knowledge of healthcare regulations and standards

• Excellent communication, organizational, and problem-solving skills

• Ability to work effectively in a fast-paced environment and prioritize tasks with multiple deadlines
