Housing Access Navigator & Assessor
Avenue-360-Health-and-Wellness
Avenue 360 is seeking an Outreach Navigator/Assessor to identify, assess, and connect individuals experiencing homelessness with housing resources through CES. You will conduct outreach, complete housing assessments in HMIS, and provide pre-navigation services to help clients obtain documentation for housing programs such as PSH and RRH. The role requires a Bachelor's degree or equivalent experience, at least two years in social services or housing outreach, and strong communication and #J-18808-Ljbffr Avenue-360-Health-and-Wellness
