Growing together.If you are in the Travis, TX area, and surrounding communities within commuting distance, you will have the flexibility to work remotely as you take on some tough challenges.Primary Responsibilities:Assess, plan, and implement person-centered strategiesConnect members with financial, psychosocial, community, and state resourcesCoordinate care across the continuum and serve as a key point of contactAdvocate for members and families to ensure their needs and preferences are heard and supportedCollaborate with providers, members, caregivers, and community partners to improve health outcomesTravel up to 75% of the time within the Mission, TX area and surrounding areasRequired Qualifications:Current unrestricted Texas RN license2+ years of experience in a community health or healthcare settingIntermediate level of proficiency with Microsoft Office applicationsHigh-speed internet at residenceAccess to reliable transportation and valid US driver's license with the ability to travel within assigned territoryPreferred Qualifications:Experience with Medicaid, LTSS, or long-term care servicesKnowledge of HCBS and integrated care principlesKnowledge of the principles of most integrated settings, including federal and State requirements like the federal home and community-based settings regulationsBilingual in English and SpanishYou will be rewarded and recognized for your performance in an environment that will challenge you and give you clear directions on what it takes to succeed in your role as well as provide development for other roles you may be interested in.
