Office & Field LPN

Care Plus Home Health Care Inc

Job Description

We are seeking a Licensed Practical Nurse (LPN) with Home Health experience to provide both office-based nursing care and independent home visits within the local area. This role involves a balanced mix of office duties and hands-on patient care in their homes, supporting patient health through medication administration, vital signs monitoring, and patient education.

Responsibilities

Conduct patient visits independently in the local area
Provide nursing care and support within the office setting
Administer medications and monitor vital signs during visits
Maintain accurate and detailed patient records
Educate patients on health management and care plans
Communicate effectively with patients and healthcare team members
Utilize standard medical supplies during home health visits

Required Qualifications

Licensed Practical Nurse (LPN) with a valid LPN license
Minimum of 1 year experience in licensed practical nursing
Experience in Home Health Care
