Family Practice/Primary Care Nurse Practitioner

We are seeking a dedicated and skilled Nurse Practitioner to join our team at WCS Healthcare Partners. This role involves providing comprehensive care to residents across two skilled nursing communities, focusing on subacute, long-term care, assisted living (including memory care), and independent living environments. The ideal candidate will have a strong background in managing complex cases and exhibit excellent communication skills in a family-facing environment.

Responsibilities

• Conduct morning rounds focusing on subacute residents and those with priority needs.
• Hold afternoon office hours dedicated to independent living patients.
• Collaborate with telehealth partners to ensure seamless after-hours coverage.
• Provide occasional coverage when the attending physician is on vacation, with compensation.

Requirements

• Certified as an FNP, ANP, or AGNP.
• At least 1 year of experience in subacute or hospital settings dealing with complex cases.
• Ability to flex across different levels of care.
• Strong communication skills in a family-facing environment.
