The Physical Therapist (PT) - Home Health is responsible for providing in-home rehabilitation services to patients with physical impairments, helping them improve mobility, function, and overall quality of life.
This role requires independent clinical judgment, strong patient interaction skills, and the ability to manage care in a home-based setting.
Must-Have Qualifications
• Minimum 1-2+ years of Physical Therapist experience (Home Health preferred)
• Valid Physical Therapist License - Iowa (Required prior to start)
• BLS Certification (AHA - Required)
• Degree in Physical Therapy (DPT or equivalent)
• Strong knowledge of rehabilitation and patient care practices
• Valid Driver's License (Required for home visits)
Preferred Qualifications
• Experience in home health or community-based care settings
• Familiarity with EMR systems
• Strong patient education and communication skills
Work Environment
• Home Health / Patient Home Settings
• Onsite / Travel-based role within Fort Dodge, IA
• Independent field-based clinical environment
Additional Requirements
• Must have reliable transportation for home visits
• Must be flexible with day shifts and scheduling needs
• Ability to manage independent caseloads
• Must comply with all clinical and regulatory standards
