About the Position

Job Title: Registered Nurse (RN) Profession: Nursing Specialty: Home Health Duration: 260 days Shift: Day Hours per Shift: 12 Experience: Two (2) years of experience in a home setting within the last ten (10) years License: Current RN Licensure in Client Certifications: Basic Life Support (BLS) required before start date Must-Have: - Experience conducting comprehensive assessments of patients - Experience working in the field for a Certified Home Health Agency (CHHA), Licensed Home Care Service Agency (LHCSA), or Managed Long-Term Care (MLTC) - Experience providing primary care to homebound persons Description: Required licensure is a current RN Licensure in Client. Required experience includes two years of experience in a home setting within the last ten years. Nurses must have recent experience conducting comprehensive patient assessments. Responsibilities include performing initial in-home pre-evaluations of patients. Evaluations will assess health status, strengths, care needs, and preferences. You will guide the development of individualized long-term care service plans. Review consumer medical documentation and health referral forms as relevant. Enter evaluation data into electronic forms and transmit as required. Document any concerns or conflicting information during the evaluation process. Emphasize continuity of care to reduce or eliminate fragmentation in treatment plans. Discuss health care options and service vendor choices with consumers. Initiate communication between relevant parties for enrollment. Enhance communication and relationships with interdisciplinary care team members. Collect quality review data and document case notes. Identify opportunities for health promotion and illness prevention. Maintain knowledge of community resources and network services for the target population. Meet all standards established for this position. Perform other duties as assigned by management. Candidates must be prepared to conduct in-field assessments.
