The RN collaborates with interdisciplinary team members to develop and implement individualized care plans and provides constant feedback to clients, client representatives, and nurses.This position responsibilities will include:Provide skilled nursing care, including wound care, medication administration, IV therapy, and monitoring of chronic/acute conditionsManage and monitor patients with complex needs (e.g., ventilators, tracheostomies, feeding tubes)Recognize early signs of deterioration and intervene appropriatelyEducate patients and family members on disease management, medications, and equipment useCoordinate care with physicians, therapists, and other healthcare providersMaintain accurate and timely documentation in electronic health recordsEnsure compliance with home health regulations and infection control practices
