Clinical Social Worker (MSW or LCSW)

Overview Our Clinical Social Worker (LCSW or MSW) will assist patients and their families/caregivers by providing education and support as they cope with a recent diagnosis. Most patients are geriatric and need understanding of local community resources and care planning. Job Type Per Diem, Territory: Morgan Hill Responsibilities Assess social and emotional factors related to the patient’s illness, medical and nursing requirements in the home. Develop and implement a plan for home social work services, including education of patients, family members and caregivers. Coordinate with other home health staff and community resources to provide an efficient integration of care. Maintain harmonious constructive working relationships with internal and external customers. Plan, organize, prioritize work, think critically, solve problems, and consult supervisor when needed. Maintain a positive and professional image; demonstrate self‑directed learning to meet professional development and/or license requirements. Ensure maintenance of all required licensure and certifications. Qualifications Master’s Degree from an accredited Council on Social Work Education program. Basic Life Support Certification – BLS required. LCSW license preferred; supervision provided to eligible Social Workers. Driver license and automobile insurance required. One year of experience as an MSW in a health‑care setting strongly preferred. Home health experience preferred. Knowledge of local community resources required. Knowledge and understanding of individual development, human behavior, illness/injury effects, and cultural influences on health care. Knowledge of state and federal home‑care regulatory guidelines preferred. Demonstrated written and verbal communication skills. Time‑management skills; ability to handle sensitive issues and conflict. Capacity to manage conflicting priorities and time demands.
