Licensed Practical Nurse

JOB SUMMARY Provide compassionate, high-quality nursing care to residents in a Long-Term Care unit, including medication administration, assessments, care planning, documentation, and coordination with physicians and the interdisciplinary care team. Key Responsibilities Administer medications and treatments as prescribed. Perform comprehensive resident assessments. Develop and implement individualized care plans. Document resident care, progress, and observations accurately. Coordinate care with physicians and the interdisciplinary team. Educate residents and families on health conditions and treatment plans. Monitor residents for changes in condition and report to appropriate personnel. Maintain a safe and clean environment for residents. Schedule / Shift Ability to work the assigned schedule, including weekends and holidays as needed.
