Physician Assistant, In-Home Visits

Overview Signify Health, part of CVS Health®, is seeking a Part‑Time Clinician (Nurse Practitioner or Physician Assistant) to conduct in‑home health evaluations. You will visit patients in the comfort of their homes, provide personalized care, and help connect them to the right health services. Responsibilities Conduct in‑home health evaluations lasting about 45 minutes, including medication and medical history review, physical examination, and any diagnostic tests ordered by the patient’s health plan (e.g., spirometry, diabetic eye exams). Use an iPad and the intuitive clinical workflow for each visit. Provide Diagnostic & Preventive Services such as spirometry, DEE, bone density screening, and others as indicated. Perform virtual visits when required. Travel locally and within licensed states as needed to reach patients. Establish and maintain trusted relationships with patients and their families. Follow all safety and procedural protocols, including working safely in environments that may contain pets, secondhand smoke, or other allergens. Work Environment Primary workplace is the patient’s private residence. Clinicians must be comfortable delivering care in diverse home settings and handling any environmental challenges. Qualifications Active, unrestricted license(s) in the coverage area(s). Board certification (required). Multi‑state licenses (preferred). For NPs and PAs: must hold valid credentials and meet all state licensure requirements. Flexibility to travel and to work virtual visits as requested. Salary & Benefits Pay Range: $41.85 – $90.13 per hour (or equivalent full‑time salary). The exact offer varies by experience, education, geography, and other factors.
