Work Environment

This position is fully remote, allowing for a flexible work schedule that supports both our team members and clients. Although you will be primarily working from home, our clinic values collaboration and communication, encouraging regular virtual meetings and opportunities for professional development. A typical workweek runs from Monday to Friday, from 8:00 AM to 4:30 PM, with the possibility of occasional evening or weekend hours to meet client needs. On-call availability may also be necessary to provide urgent support during times of crisis.

Key Responsibilities

• Conduct comprehensive assessments of clients' psychosocial needs, considering emotional, mental, and social factors that may impact their well-being.
• Collaborate with clients and their families to create individualized care plans that prioritize their specific goals and preferred methods of support.
• Provide counseling and emotional support to clients, assisting them in navigating significant life transitions and challenges, ensuring that they feel heard and understood.
• Advocate for clients’ rights and facilitate access to community resources, ensuring they receive the necessary support and services for their unique circumstances.
• Work within interdisciplinary teams, including healthcare professionals and caregivers, to coordinate care and enhance the overall treatment strategy.
• Conduct regular check-ins with clients to monitor their progress, assess living conditions, and address any concerns regarding safety or mental health.
• Foster communication among family members to resolve conflicts, ensuring a unified approach to the client’s care.
• Educate clients and families about available community resources, support groups, and programs that can enhance their social engagement and improve their quality of life.
• Maintain meticulous documentation of client interactions, assessments, and care plans to ensure continuity and compliance with organizational standards.
• Stay current with best practices in social work, including changes to policies and regulations, to maintain ethical standards in your practice.

Patient Population / Client Focus

Our primary focus includes seniors and their families, who often face unique emotional and social challenges. This role emphasizes the importance of understanding geriatric care, recognizing the myriad of issues associated with aging—including depression, anxiety, and adjustment to life changes. By working closely with clients, you will help them navigate these challenges and find ways to foster resilience and independence.

Qualifications

• Master's degree in Social Work from an accredited institution with a strong foundation in clinical practice.
• Active and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• At least two years of relevant experience in a social work role, specifically within community health or geriatric settings.
• Profound understanding of social work principles, aging-related issues, and familiarity with community resources available to clients.
• Exemplary communication and interpersonal skills that promote trust and rapport with clients.

Preferred Experience

• Experience in telehealth or providing remote mental health services is highly desirable.
• Knowledge of mental health counseling techniques and evidence-based practices for anxiety and depression.
• Proficiency in utilizing electronic health record systems to maintain client documentation.
• Previous work in interdisciplinary teams and familiarity with collaborative practice approaches.
