This position supports students in accessing appropriate campus and community-based services through assessment, referral, care management, response, and transition care while collaborating with mental health counselors, campus partners and community providers to promote student well-being and continuity of care.

The Senior Coordinator, Mental Health Clinical Case Manager provides direct clinical services, intensive case management, and care coordination for students experiencing mental health, psychological, and developmental challenges. This position supports students in accessing appropriate campus and community-based services through assessment, referral, care management, response, and transition care while collaborating with mental health counselors, campus partners and community providers to promote student well-being and continuity of care.
Job Description:
This position is 100% remote with on-campus presence, as needed. Description of Duties and Tasks- Serves as a clinical case manager for students with mental health concerns. Responsibilities include assessing student needs, referral information, coordinating services, and maintaining records. The position ensures ongoing support through follow-up and case management, and assisting the mental health counseling services on complex cases.
- Provide consultation and outreach services to students, campus and community members related to mental health issues and student concerns.
- Assist students with coaching and skills training on self-advocacy, navigating health insurance, and connecting with mental health, and other wellness services.
- Maintain accurate and confidential records of all referrals, follow-ups, and service connections.
- Manages data systems to track services and outcomes, produces reports for program evaluation, and analyzes trends to recommend improvements or guide resource and service planning.
- Fosters collaboration with campus departments and community agencies to connect students with resources, stays informed on best practices through meetings and trainings, and represents the mental health counseling team on committees and working groups as assigned.
- Provide outreach presentations to the campus community, as needed.
- Leads development of comprehensive project plans for small to medium-sized initiatives, showing attention to detail and effective time management.
- Stakeholder Collaboration: Demonstrates strong coordination skills by effectively engaging with mental health counselors, college personnel and external partners.
- Operational Support: Oversees coordination of special, innovative departmental initiatives, demonstrating a commitment to organizational efficiency.
- Provide consultation and outreach services to students, campus and community members related to mental health issues and student concerns.
- Assist students with intake paperwork, processes, and gather preliminary intake information, as needed.
Knowledge
- Principles of student mental health services and how to connect students with appropriate resources.
- Data tracking, analysis, and reporting techniques to evaluate program effectiveness and identify trends.
- Familiarity with crisis intervention and response strategies.
Skills
- Strong organizational, planning, and time management skills with the ability to prioritize tasks.
- Effective communication skills, including the ability to explain complex processes clearly and empathetically to students.
- Ability to build relationships with counselors, community partners, and campus departments.
- Strong problem-solving skills, particularly in addressing and meeting students' needs.
- Attention to detail and accuracy in documentation and ability to maintain confidentiality and handle sensitive situations with discretion.
- Team-oriented with the ability to work independently when needed.
Technology Skills
- Proficiency in Microsoft Office Suite (Excel, Word, PowerPoint) and data management tools.
- Experience with student information systems, assessment tools, and service tracking platforms.
- Familiarity with data analysis and reporting software.
Required Work Experience
- Two (2) of related work experience working in student services, social services, or case management.
Preferred Work Experience
- Minimum three (3) years of experience working in student services, social services, orrelated support roles.
- Experience working with case management or referral tracking systems.
- Experience in a higher education setting, particularly with under-served student populations.
Education
- Bachelor's degree.
- Four (4) years of related experience, or an Associate's degree with two (2) years of related experience may be substituted for the required education (plus the required experience noted below).
Preferred Education
- Master's Degree in Social Work, Counseling, or a related field.
Special Requirements
- State license as a mental health provider (e.g., LCSW, LPC)
- Bilingual or multilingual (especially Spanish or other locally spoken languages) is preferred.
Physical Requirements
- Work is performed in a standard office environment.
- Subject to standing, walking, sitting, bending, reaching, pushing, and pulling.
- Ability to lift objects up to 20 pounds.
