Outpatient RN Care Manager

The Registered Nurse Care Manager will case manage participants and their medical needs in the clinic and in the home for primary care needs. Support the provider, do initial and reassessments as per regulations. Assist with level of care determination assessments required to determine eligibility for the program. Conduct some lab procedures. Assist with medication management. Participate in care planning and be part of the interdisciplinary team. Daily patient ratio is 1:40. Oasis experience is required. Candidate is expected to have reliable transportation and no additional scheduling restrictions. All travelers are expected to float between all facilities within a given state. Travelers' refusal to float may result in termination of assignment. Providers will be responsible for submitting monthly cell phone bills directly to the agency.
