Community Partnerships Manager, Field-Based (Healthcare)

People Care Inc. is seeking a Community Relations Manager to expand partnerships with MLTC plans, hospitals, and community organizations across New York. This is a field-forward role focused on real-world outreach and trust-building in diverse neighborhoods. You’ll represent the company at health fairs and partner meetings, develop culturally aware outreach strategies, and work closely with Intake, Coordination, and Clinical teams to convert referrals into care.
