Clinical Liaison (CL) PRN

Clinical Liaison – Inpatient Rehabilitation Per Diem Including Weekends The Clinical Liaison plays a pivotal role in ensuring the continuum of care for patients within inpatient rehabilitation. They maintain referral relationships, coordinate intake, and evaluate patient suitability for admission. How You'll Contribute Build and maintain referral relationships to drive appropriate admissions to the hospital. Assist and coordinate the intake and pre‑admission screening process. Perform on‑site clinical judgment to determine the appropriateness of the patient for admission. Communicate rehabilitation and facility options to patients and families, determining admission appropriateness and assessing patient expectations and requirements for participation. Secure information relating to patients' resources and benefits. Act as initial contact for external case management and discharge planner personnel with payors and hospital facilities. Previous marketing/sales experience preferred. Previous clinical experience preferred, with demonstrated skills in clinical assessment and EMR systems. Formal sales training preferred.
