Pay $21.97 per hour Schedule Every Monday and Friday 7:00 am–3:30 pm and Saturday and Sunday every other weekend 7:00 am–3:30 pm Requirements Valid driver’s license for at least 1 year post‑permit and a clean motor vehicle record meeting Sevita’s insurability policy. Must be 18 years of age or older. High school diploma or equivalent preferably. Six months of experience in human services preferable. Working knowledge of computers. Valid driver’s license in good standing (as required for transportation). All state-required training and certification completed in mandated timeframes. Responsibilities Provide direct support within a single program or unit, typically supporting 4‑6 individuals in residential settings or up to 10 individuals in periodic programs. Deliver support according to individual service or program plans—including socialization, health maintenance, medication administration, and skill/behavioral development. Assist with daily activities: personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living. Accurately document progress and activities; review records and logs to stay abreast of service plan changes; maintain confidentiality. Make cost‑effective purchases within the budget of the individual served. Maintain healthy professional relationships with individuals, families, guardians, and case managers; greet family members and visitors; implement Sevita Customer Service Standards. Report abuse or neglect and other incidents to internal and external authorities as required. Practice universal precautions and respect the rights of the persons served. Accompany individuals to medical appointments; relay orders and information as needed. Accurately administer or support self‑administration of medication; document delivery; promptly report errors; maintain security of controlled substances. Monitor health concerns; communicate with nurses or supervisors; call 911 for emergencies; follow individualized health care directives. Prepare or support meal preparation according to menus and dietary preferences; monitor mealtime. Attend orientation and ongoing training; participate in monthly staff meetings. Work scheduled hours, arrive on time, stay on shift until replacement arrives; notify supervisor when unable to work. Support teamwork, show respect, listen and communicate openly. Promote workplace safety; comply with safety policies and report hazards. Drive safely, use safety equipment, report accidents immediately. Perform daily and seasonal housekeeping; report hazards. Monitor environmental safety and perform minor repairs as needed; report maintenance issues. Check water temperature and participate in safety drills. Perform additional duties as required. Other Duties Mobile Punch Authorized for timekeeping. Supervisory Responsibilities None. Work Environment Works at the program location and may accompany individuals into the community. Physical Requirements Heavy work: Exert up to 100 pounds occasionally, up to 50 pounds frequently, up to 20 pounds constantly. Must have a valid driver’s license in good standing in the state of residence. All employees hired after 6/15/2018 must submit to drug testing prior to employment and subsequently on a random basis. Employees must submit to criminal background checks and be cleared for service with state registries. Must complete all required training and demonstrate proficiency. Minimum age 18 years.
