RN/LPN- Home Care

Job Summary

We are seeking compassionate and skilled Registered Nurses (RN) and Licensed Practical Nurses (LPN) to provide one-on-one private duty care in a home setting. The nurse will deliver high-quality, patient-centered care to individuals with complex medical needs, ensuring comfort, safety, and dignity.

Key Responsibilities

• Provide direct nursing care in the patients home
• Administer medications and treatments as prescribed
• Monitor and document patient condition and vital signs
• Perform skilled nursing procedures (e.g., trach care, ventilator management, G-tube feeding, wound care)
• Collaborate with physicians, families, and care teams
• Educate patients and families on care plans and disease management
• Maintain accurate and timely documentation

Qualifications

• Active RN or LPN license in Massachusetts
• Current CPR/BLS certification (required)
• Minimum 1 year nursing experience (home care, pediatric, or critical care preferred)
• Experience with trach/vent, G-tube, or complex care (preferred but willing to train)
• Strong clinical and communication skills
• Reliable transportation and ability to travel to patient homes
