POSITION SUMMARY - RN Case Manager


POSITION DUTIES - Coordinate patient care from admission through discharge, ensuring that treatments, services, and resources align with each patient's medical needs and recovery goals. Collaborate closely with physicians, nurses, and external providers to remove barriers to care, streamline transitions, and support safe, efficient discharge planning.


MINIMUM REQUIRED QUALIFICATIONS -

• ME/compact RN license
• BLS
• 2+ years recent case management experience in an acute hospital setting
• Skilled experience in discharge planning and care coordination

PREFERRED QUALIFICATIONS - 5+ years recent case management experience in an acute hospital setting


LENGTH OF ASSIGNMENT - 13 weeks


SHIFT / HOURS PER WEEK - M-F 7:30a-4p plus every third weekend


SYSTEMS - Epic
