In this role, you’ll have the unique opportunity to provide comfort, dignity, and support to individuals facing advanced illness—while working alongside a collaborative, mission-driven team. Your schedule would be: 8:30am - 5pm & every other weekend.Since 1978, Trustbridge Hospice has been honored to serve families across Palm Beach and Broward counties with compassionate, community-based care. Formed from the legacy of Hospice by the Sea and Hospice of Palm Beach County, we continue to provide comfort, dignity, and peace to those facing advanced illness.**What you'll Do**As an RN Case Manager, you will play a vital role in coordinating care and supporting patients and families throughout their hospice journey.
