Compensation: $37.00 to $61.74 USD per hour (Exact rate is determined on a case-by-case basis commensurate with experience level, as well as education and certifications pertaining to each position which may be above the listed job requirements.) - Coordinate care with patients, families, physicians, and interdisciplinary team members to promote positive clinical outcomes and seamless transitions of care.
- Monitor patient conditions, implement appropriate interventions, and adjust care plans based on changing needs and treatment goals.
- Educate patients and families regarding diagnoses, treatment plans, medications, self-care, and discharge instructions.
- Utilize electronic health record systems to document care accurately and maintain compliance with organizational standards.
- Participate in interdisciplinary rounds, communicate effectively with team members, and ensure safe patient handoffs.
- Delegate and supervise nursing support staff appropriately while maintaining accountability for patient care delivery.
- Support a culture of safety, teamwork, quality improvement, and professional excellence through active participation in unit and organizational initiatives.
- Licensure: Active, unrestricted New York State (NYS) RN License.
- Certifications : Basic Life Support (BLS), Crisis Intervention Team (CIT), and NAPPI certifications are required and fully provided by the hospital during your comprehensive onboarding process.
